With all the debate surrounding the vaccination of children, we decided to look into the the issue from a unique angle. Below you will find a short eleven question survey of the major issues surrounding the debate. All responses are collected anonymously via Google Forms, but you will need to be logged into your Google account to see and fill out the survey.
